The day supply nobody wrote down

You are the owner, and four days a week you are also the only pharmacist on duty.

The voicemail

From memory

Fundamentals 6

A few words is enough and nothing here is marked. If it has gone, say so and you still get the answer.

Whose chair: yours, at the computer, before the first patient. Or your billing clerk's, who will pick this up if you do not. Below is the voicemail as it was transcribed. Everything in it is invented, and so is the drug.

Ashgrove Family Pharmacy does not exist. Neither does wolkabin, nor Kelvarn Laboratories, nor the prescriber. The DEA number and the NPI printed later are deliberately invalid: each one fails its own check digit, so neither can belong to a real registrant.
VOICEMAIL, 8:40 AM, Tuesday. Transcribed by the phone system. Hi, this is Carol calling about my mom's blood pressure tablets, the wolkabin ones. She says she still has two refills left on it. Could we pick them up before Thursday? She thinks she is nearly out. Thanks.

Two claims arrived in nine seconds. There are two refills left, and she is nearly out. One of them is checkable from a document you hold. The other is a claim about a cupboard in somebody's kitchen, and no document in your building settles it.

It is 8:40 on a Tuesday and there is a voicemail from a patient's daughter about her mother's blood pressure tablets. She says there are two refills left and she would like them before Thursday. This is the lesson where the tool agrees with her twice, in a paragraph you could paste straight into a text message, and is wrong both times.

Lesson 01 · The person at the window

By the end you can

  • Name the one document that settles a sentence about a specific fill, and read only the fields that sentence depends on
  • Compute a day supply from the sig and the quantity instead of reading a number off the page
  • Run the Hard Copy Check on any AI drafted sentence before it reaches a patient

What you need

Any AI tool, free tier. Nothing to upload. The voicemail and the hard copy are both on this page and both are invented. You will want your own state board's website open in a second tab at the end.

What it does with the voicemail alone

4m

Start where a busy morning starts: hand the tool what you have and ask for the reply. This step is a demonstration and not a trap. Watch what it does when it has almost nothing.

Paste this, the voicemail included

Write a short, friendly text message back to this patient's daughter from our pharmacy, confirming her mother's refill and when she can collect it.

Carol's voicemail

Most tools are careful here, and a careful reply is the point of the step rather than a disappointment. Given a voicemail and nothing else, a tool often returns a template with blanks in it, because there is nothing to be confident with. Notice what it did do: it took Carol's word for the two refills and passed it straight back. It did not invent that. It repeated it. That is the shape of the whole problem, and it gets worse in a moment, not better, when you give it more.

The whole face of the paper

5m

Now pull the hard copy. This is the prescription as your pharmacy holds it, and it is the document that settles what was authorized, by whom, and on what date. It is the paper a board inspector asks for and the paper a PBM auditor asks for. Read the whole face.

HARD COPY, as filed. Rx 0000001. FICTIONAL DOCUMENT: the pharmacy, the prescriber, the patient and the drug do not exist, and the identifiers below are deliberately invalid. Patient: C. Whitfield Prescriber: R. Raman, MD DEA BQ1234561 NPI 9876543210 Date written: 8 August 2026 wolkabin 20 mg tablets (Kelvarn Laboratories) Sig: take 2 tabs BID Quantity: 30 Refills: 2

Guess first

You are about to hand the tool that whole page and ask it to write the fill note. What will it say the day supply is?

And how sure are you?

Nobody else sees this and nothing here is marked. Being wrong here is worth more than being right.

Paste this, the hard copy included

Here is the prescription. Write the internal fill note for our records and a short text back to the patient's daughter. Include the day supply.

The hard copy

Patient: C. Whitfield Prescriber: R. Raman, MD DEA BQ1234561 NPI 9876543210 Date written: 8 August 2026 wolkabin 20 mg tablets Sig: take 2 tabs BID Quantity: 30 Refills: 2

Two tablets twice a day is four tablets a day. A quantity of 30 is seven and a half days. The number 30 on that page is a quantity, and it is sitting one line above a sig that turns it into a week. Every field the answer needed was on the page, in front of it, and it still read the wrong one, because 30 is the number that looks like a day supply in almost every prescription it has ever seen.

THE HARD COPY CHECK Before any AI drafted sentence about a specific fill, patient, claim or duty leaves the building, pull the hard copy and read the fields that sentence depends on. If the sentence cannot be traced to a line on the paper, it is not a fact yet. When the sentence is about the law rather than a fill, the hard copy is the rule text, its citation and its effective date. The check happens on the paper, away from the tool. Nothing off a hard copy, a profile or a monitoring program printout gets typed or photographed into anything.

The document it was never given

3m

One claim is still standing: the two refills. It is on the face, in Carol's voicemail, and now in the draft. Here is the dispensing history, which is a different document with a different custodian.

DISPENSING HISTORY, Rx 0000001. Fictional, as above. 8 August 2026 original qty 30 17 August 2026 refill 1 qty 30 26 August 2026 refill 2 qty 30

Refills: 2 on the face is the number the prescriber authorized. It is not the number remaining, and remaining is not on the hard copy at all. Both refills have been dispensed. There is nothing left to fill, and Carol is right that her mother is nearly out, which makes this a call to the prescriber's office rather than a bag on the shelf. Look at the fill dates while you are here: nine days apart, then nine days apart. The paper has been telling you the day supply for a month.

That is the Hard Copy Check run once, on one prescription, in about five minutes. The tool did not fail because it lied or because you starved it. It failed with the whole page in front of it, on a number that was never written down, and then it repeated a patient's own words back to you as though they were a record. Every lesson in this track is that shape with a different piece of paper: a claim, a reject, a prior authorization, an audit letter.

Where that leaves you

You want to hand the next one over properly, and that is where the real trouble starts. The profile your system holds carries her name, her street address, her date of birth, her cardholder ID and eleven prescription numbers, and the gate on putting any of that into a tool is not an AI rule at all.

That was lesson 1 of 30. A free account saves your progress and shows you what the other 29 lessons cover.
